Reconstructive Breast Surgery Benefits. Reconstructive breast surgery resulting from a mastectomy is covered. Coverage includes reconstruction of the breast on which the mastectomy is performed including areolar reconstruction and the insertion of a breast implant; surgery and reconstruction performed on the non-diseased breast to establish symmetry when reconstructive breast surgery on the diseased breast has been performed; and Medically Necessary physical therapy to treat the complications of mastectomy, including lymphedema.

Reconstructive Breast Surgery Benefits. Reconstructive breast surgery resulting from a mastectomy is covered. Coverage includes reconstruction of the breast on which the mastectomy is performed including areolar reconstruction and the insertion of a breast implant; surgery and reconstruction performed on the non-diseased breast to establish symmetry when reconstructive breast surgery on the diseased breast has been performed; and Medically Necessary physical therapy to treat the complications of mastectomy, including lymphedema. As an exception to the Medically Necessary requirements of this Certificate, the following coverage is provided following a mastectomy for a length of time determined by the attending Physician to be Medically Necessary. Coverage will include one post-discharge in-home nurse visit or Physician office visit, by a licensed Provider, to verify the condition of the Member in the first 48 hours after discharge.

  • Dental Benefits The County offers dental and orthodontic benefits to full and part-time regular employees and their eligible dependent(s).   • Medical Benefits The Company shall reimburse the Employee for the cost of the Employee's group health, vision and dental plan coverage in effect until the end of the Termination Period. The Employee may use this payment, as well as any other payment made under this Section 6, for such continuation coverage or for any other purpose. To the extent the Employee pays the cost of such coverage, and the cost of such coverage is not deductible as a medical expense by the Employee, the Company shall "gross-up" the amount of such reimbursement for all taxes payable by the Employee on the amount of such reimbursement and the amount of such gross-up.
