Notice and Submission of Claims. Within twenty-one (21) days after receiving the Class Member Lists from PSI Defendants and CPS Defendants (as described in Section V. B.) or within twenty-one (21) days after the Preliminary Approval Date, whichever is later, the Settlement Administrator shall send each Class Member a copy of the Notice of Settlement, attached hereto as the first page of Exhibit A, via First Class regular U.S. mail. The Notice will be mailed using the most current mailing address information on file with CPS Defendants and/or PSI Defendants as provided to the Settlement Administrator.

Claim Forms shall be available from Class Counsel, the Settlement Administrator, on the Settlement Website, and in the Rutherford County Courthouse and Rutherford County Probation Office. To file a Valid Claim, each eligible Settlement Class Member must: (i) complete a Claim Form, providing materially all of the information and documentation required by the Settlement Agreement and the Claim Form; (ii) sign the Claim Form and state under penalty of perjury that he or she believes that he or she is a member of the Settlement Class and that the information submitted is true and correct; and (iii) return the completed and signed Claim Form to the Settlement Administrator no later than one hundred twenty (120) Days after the Preliminary Approval Date, i.e., the Claim Deadline. Only eligible Settlement Class Members who submit Valid Claims shall be entitled to a Cash Award.
