Hospital Authority No. 1 of Lancaster County, Nebraska (the “Lancaster Authority”) previously issued $4,500,000 principal amount of its Healthcare Revenue Bonds (Xxxxxxx, Inc. Project), Series 2009A (the “Series 2009A Bonds”), pursuant to a Trust Indenture dated June 30, 2009 (the “0000X Xxxxxxxxx”), between the Lancaster Authority and Xxxxx Fargo Bank, National Association, a national banking association duly organized and existing under the laws of the United States of America and having a corporate trust office located in Lincoln, Nebraska, as trustee (the “Bond Trustee”), the proceeds of which were loaned (the “2009A Loan”) to the Corporation pursuant to a Loan Agreement dated June 30, 2009 (the “2009A Loan Agreement”) between the Lancaster Authority and the Corporation.

Hospital Authority No. 1 of Saline County, Nebraska (the “Saline Authority”) issued $4,030,000 principal amount of its Healthcare Revenue Bonds (Xxxxxxx, Inc. Project), Series 2010A (the “Refunded Bonds”), dated January 6, 2010, pursuant to a Trust Indenture dated January 6, 2010 (the “0000X Xxxxxxxxx”), between the Saline Authority and the Bond Trustee, the proceeds of which were loaned (the “2010A Loan”) to the Trustor pursuant to a Loan Agreement dated January 6, 2010 (the “2010A Loan Agreement”) between the Saline Authority and the Trustor. To evidence the 2010A Loan and to provide for the payment of the Refunded Bonds the Corporation executed and delivered its Promissory Note dated January 6, 2010 in the principal amount of $4,030,000 (the “2010A Note”).

  • Medical Authorization In the event of illness or injury while participating in the above referenced activity, I hereby consent to whatever x-ray, examination, anesthetic, medical, surgical, dental diagnosis or treatment, hospital care and emergency transportation from a licensed physician, surgeon, and/or dentist as deemed necessary for my safety and welfare.

  • Final Authority In case of dispute with respect to credits or deductions, the decision of the Board shall be final subsequent to prior consultation between the employee concerned and the administrative officials.

  • Competent Authority Exchange of information concerning Competent Authority Requests is expected to take place between the Competent Authorities or their authorized representatives. The Competent Authorities intend to provide written notification to each other providing contact information (name, title, and mailing address) for each jurisdiction’s Competent Authority for purposes of the IGA, and to update this contact information, as appropriate.

  • Local Authority Central Spend Equivalent Funding representing a proportion of the LA Education Budget money which the LA would be able to retain, from the non- delegated elements of the Schools Budget and the relevant items in the LA Budget, if the Academy were a maintained school. The proportion which this funding will represent will be based on the elements of the LA's Section 251 Budget Return which are relevant to that Academy.

  • Hospital and Medical Insurance The University shall make available health insurance to the employees covered by this agreement to the same extent and in the same manner as is available to other University employees, such as Faculty and the Executive, Administrative and Professional Staff employees. It is the University's goal to have the same health insurance plans offered uniformly to all University groups and employees.

  • LEGAL AUTHORITY The Contractor warrants and assures H-GAC that it possesses adequate legal authority to enter into this Agreement. The Contractor's governing body, where applicable, has authorized the signatory official(s) to enter into this Agreement and bind the Contractor to the terms of this Agreement and any subsequent amendments hereto.

  • Governmental Entity “Governmental Entity” shall mean any federal, state, local or foreign court, arbitral tribunal, administrative agency or commission or other governmental or regulatory authority or administrative agency.

  • Competent Authorities The Parties shall inform each other about the structure, organisation and division of competences of their competent authorities during the first meeting of the Sanitary and Phytosanitary Sub-Committee referred to in Article 65 of this Agreement ("SPS Sub-Committee"). The Parties shall inform each other of any change of the structure, organisation and division of competences, including of the contact points, concerning such competent authorities.

  • General Authority The Owner Trustee is authorized and directed to execute and deliver the Basic Documents to which the Trust is to be a party and each certificate or other document attached as an exhibit to or contemplated by the Basic Documents to which the Trust is to be a party and any amendment or other agreement or instrument described herein, as evidenced conclusively by the Owner Trustee's execution thereof. In addition to the foregoing, the Owner Trustee is authorized, but shall not be obligated, except as otherwise provided in this Trust Agreement, to take all actions required of the Trust pursuant to the Basic Documents.

Residential Treatment Facility This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. Intermediate Care Services This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.
