Common use of Group Life and Accidental Death and Dismemberment Clause in Contracts

Group Life and Accidental Death and Dismemberment. benefits each shall be set at three (3) times the employee’s annual salary to a maximum of $250,000. The premiums for the Life and Accidental Death and Dismemberment Insurance Plans are paid by the Employer. The Employer agrees to make available optional voluntary life insurance (maximum $200,000) for eligible faculty members and their spouse/common-law spouse, subject to the applicant meeting insurance company requirements. All premiums for this optional life insurance will be paid by the faculty member. Life insurance benefits will be paid in accordance with the schedule of benefits listed in the insurance carrier's plan and are subject to the limitations specified in the plan including eligibility requirements.
