FREQUENCY & PROCESS. In addition to the evaluations set forth in the probationary period, all Professional staff shall be evaluated at least once every two (2) years. The WEAC Department Manager or appropriate entity in the UniServ shall evaluate the staff in their Department or UniServ. Such evaluations shall include a conference between the staff person and the evaluator followed by a written report. The staff person shall have an opportunity to attach comments to the written report. The written report will be placed in the employee’s permanent personnel file.

FREQUENCY & PROCESS. The panel will meet on a three weekly basis to review the papers provided at Guildford and Waverley CCG or Borough Council offices. All paper work must be submitted to the panel administrator no later than 4pm 5 days before panel. Applications should be sent to: S117 panel administrator Secure email: xxxxx.Xxxxxxx000@xxx.xxx All applications must be submitted with a fully completed application form, a S117 aftercare plan with clearly identified after-care needs, crisis contingency plan and supporting evidence for placement or other support proposed for panel consideration:  Responsible commissioner evidenced  Care coordinator following discharge identified  Care planning must be clear and fully detailed regarding meeting the needs which arise from the individual’s mental illness  Must identify services aimed at preventing relapse or admission from the individual’s mental illness  Must identify how those needs will be met and the likely outcome for the person and any crisis and contingency planning  Evidence of clinical effectiveness or relevant NICE recommendation  Clearly identified costs Poor quality applications or applications that do not contain all the relevant information will be returned to the care coordinator making the application.
