Common use of Eyeglass Plan Clause in Contracts

Eyeglass Plan. The Township will provide three hundred ($300) dollars every two (2) years to any active and retired Employee covered under any Township offered health insurance plan for the Employee, spouse and covered dependents to the maximum of $1,200.00 every two (2) years. Employees will be reimbursed through the Township's purchasing system. Employee’s normal allotment may be applied to cost of Laser Eye surgery for the Employee only.

The Township will provide $250.00 every two (2) years for those employees, depending upon date of hire, covered under the Township self- insurance health plan for the Employee, spouse and covered dependents to the maximum of $1,000.00 every two (2) years.
