DMO Dental. During the term of this agreement, the City agrees to provide dental benefits for employees and their dependents. DENTAL DMO Employer Pays Single: 100% EE+1: 69.8 % Family: 42.1% Section 2. PPO Dental For employees electing the PPO Dental Plan, the City will pay the same dollar amount for single, employee + 1, and Family as it pays for participants in the DMO Dental Plan. The balance of the premium shall be paid for by the employee. For example, if the City pays $11.25 per month for single participating in the DMO Dental Plan, the City shall contribute $11.25 per month to offset premiums for single participants in the PPO Dental Plan. The balance of the premium for the PPO Dental Plan shall be paid by the employee. If the DMO rates increase during the term of this agreement and the City begins to pay $14.00 per month for single employees electing the DMO plan, the City will pay $14.00 per month for employees who elect the PPO Dental Plan with the balance of the premium being paid by the employee.

  • Dental specific medications for dental purposes, including fluoride medications (except for children less than five years of age with a non-fluorinated water supply);

  • Vision The Employer agrees to offer group vision insurance to bargaining unit employees and their dependents, at employee cost.

  • Dental Care Plan The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred in respect of the coverages summarized in Appendix "1". The Plan will not duplicate benefits provided now or which may be provided in the future by any government program.

  • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate. This plan also covers facility fees associated with these services.

  • Pueblo scholarship This articulation transfer agreement replaces all previous agreements between ACC and CSU-Pueblo in Bachelor of Social Work. This agreement will be reviewed annually and revised (if necessary) as mutually agreed.

  • Dental Care a. Dental Care for Members over age 19 is limited to the following:

  • Dental Care Benefits (a) The Employer shall provide such regular, full-time seniority employee (and her eligible dependents*) the 100/75/50 Co-Pay Dental Plan in effect January 1, 2014, subject to such terms, conditions, exclusions, limitations, deductibles, co-payments and other provisions of the plan. The Employer shall pay 95% of the illustrated premium cost of such benefits and the employee shall pay the balance. Coverage shall commence on the day following the employee's ninetieth (90th) day of continuous employment.

  • Medical Flexible Spending Arrangement A. During January 2020 and again in January 2021, the Employer will make available two hundred fifty dollars ($250) in a medical flexible spending arrangement (FSA) account for each bargaining unit member represented by a Union in the Coalition described in RCW 41.80.020(3), who meets the criteria in Subsection 28.7(B) below.

  • Medical/Dental Expense Account The Employer agrees to allow insurance eligible employees to participate in a medical and dental expense reimbursement program to cover co- payments, deductibles and other medical and dental expenses or expenses for services not covered by health or dental insurance on a pre-tax basis as permitted by law or regulation, up to the maximum amount of salary reduction contributions allowed per calendar year under Section 125 of the Internal Revenue Code or other applicable federal law.
