Cardiac Rehabilitation Sample Clauses

Cardiac Rehabilitation. Outpatient cardiac rehabilitation, combined with chiropractic services, outpatient pulmonary rehabilitation, and outpatient rehabilitative physical, occupational and speech therapies, is limited to 35 visits per calendar year. Cardiac rehabilitation requires Prior Authorization.
Cardiac Rehabilitation. Medically Necessary Phase I Cardiac Rehabilitation is covered in an inpatient setting. Medically Necessary Phase II Cardiac Rehabilitation is covered on an outpatient basis for up to 36 visits per cardiac episode per Member per Calendar Year.
