Service Confirmation Form definition

Service Confirmation Form means the form (as published on the Terminal Operator’s website) duly completed by Terminal User and accepted by Terminal Operator, in accordance with the ACTL. Any duly approved Service Confirmation Form shall become part of the LTL and be added in attachment A of the LTL
