Medication therapy management services definition

Medication therapy management services means a distinct service or group of services offered by licensed pharmacists, physicians licensed to practice medicine in all its branches, advanced practice registered nurses authorized in a written agreement with a physician licensed to practice medicine in all its branches, or physician assistants authorized in guidelines by a supervising physician that optimize therapeutic outcomes for individual patients through improved medication use. In a retail or other non-hospital pharmacy, medication therapy management services shall consist of the evaluation of prescription drug orders and patient medication records to resolve conflicts with the following:
Medication therapy management services means the provision of direct patient care services by a pharmacist licensed under title 63, chapter 10 practicing in this state, to optimize the therapeutic outcomes of the patient's medications. Medication therapy management encompasses a broad range of professional activities and responsibilities within the licensed pharmacist's scope of practice. Medication therapy management services are independent of, but can occur in conjunction with, the provision of a medication product.
