Van Coevorden may cancel your participation in the Concierge Program at any time by providing at least 30-days advance written notice to you at your address provided in this Agreement.
In addition, Moorings Park may discontinue the Concierge Program at any time.
You must notify the Concierge Program as soon as possible of any changes in the information listed above.
You understand that this Agreement is a membership contract for the Concierge Program only, and is not a contract for insurance or the provision of medical services, and is not a substitute for insurance or other health plan coverage.
In the event the Concierge Program discontinues, Moorings Park will take reasonable steps to transfer your care to another physician and will refund a pro-rated portion of the Annual Fee based on the number of days that you have participated in the Concierge Program.
Please provide us with the following information about yourself and any other family members who will be Concierge Program participants.
Psychology VIP may terminate this Agreement and your participation in the Program upon 30 days prior written notice to you – if any of the following occur: (1) you fail to pay the Fees or charges for health care services when due, or (2) you fail to abide by the policies of Dr. ▇▇▇▇▇ and/or the Psychology VIP Concierge Program, or (3) if Dr. ▇▇▇▇▇ refers you to an alternative provider.
Any notice between you and the Concierge Program with respect to the terms of this Membership Agreement (such as renewal or cancellation notices) must be in writing and sent by mail or fax or hand-delivery.
You acknowledge that you are not entitled to receive any medical services from the Concierge Program as a result of your execution of this Agreement.
The Concierge Program provides premier service amenities in partnership with NCH Physicians Group.
