Staff Coverage Rates. The 2019 monthly Contributing Employer contribution rates have been provided to the Contributing Employer in the Plan’s Staff Coverage Form, the terms of which are incorporated herein by reference. Upon renewal, extension of or other changes the Plan’s medical coverage policy with its carrier, the Contributing Employer will be advised in writing of the contribution rates for the successive calendar or policy year(s) by means of a new Staff Coverage Form for each new calendar or policy year. The Contributing Employer may continue to elect staff coverage or decline to elect staff coverage by so indicating on the Staff Coverage Form and returning it signed to the Plan within the time specified by the Plan. The terms of an executed Staff Coverage Form shall be incorporated in this Participation Agreement by reference instead of a successor participation agreement, unless the Plan requires execution of a successor participation agreement. Failure to timely return the executed Staff Coverage Form or successor participation agreement for any renewal period shall be treated by the Plan as a Contributing Employer election to decline to renew and continue staff coverage. Failure to timely deliver the fully executed Staff Coverage Form or successor Participation Agreement to the Plan will cause termination of the Contributing Employer’s staff coverage at the conclusion of the current policy year and as indicated in the Staff Election Form. Once staff coverage has been Revised November 9, 2018 12 elected for a calendar or policy year, it may only be terminated effective at the end of a calendar month, and on thirty (30) days prior written notice to the Plan. Additional elements of Staff coverage including benefits and eligibility are governed by the Plan’s Rules as they may be amended by the Board of Trustees from time to time.

Upon renewal, extension of or other changes to the Plan’s medical coverage policy with its carrier, the Contributing Employer will be provided details of the contribution rates for the successive calendar or policy year(s) in the Plan’s successive Employer Benefit Enrollment Guide. Failure to timely complete and submit the Plan’s designated initial and annual enrollment processes for any renewal period shall be treated by the Plan as a Contributing Employer election to decline to renew and continue staff coverage and will cause the termination of the Contributing Employer’s staff coverage at the conclusion of the current policy year. Once staff coverage has been elected for a calendar or policy year, it may only be terminated effective at the end of a calendar month, and on thirty (30) days prior written notice to the Plan. Additional elements of Staff coverage including benefits and eligibility are governed by the Plan’s Rules as they may be amended by the Board of Trustees from time to time.

  • Mileage Rates The mileage rate shall be the maximum allowed by the Internal Revenue Service. If the IRS rate should change during the term of the contract, the contract rate shall change also on the date specified by the IRS.

  • Debt Service Coverage Ratio Borrower shall maintain as of the last day of any fiscal quarter a Debt Service Coverage Ratio of not less than 1.25 to 1.00 for the period of four consecutive fiscal quarters then ended on such day.

  • Minimum Wage Rates It shall include, in all contracts in excess of $2,000 for work on any projects funded under this grant agreement which involve labor, provisions establishing minimum rates of wages, to be predetermined by the Secretary of Labor, in accordance with the Xxxxx-Xxxxx Act, as amended (40 U.S.C. 276a-276a-5), which contractors shall pay to skilled and unskilled labor, and such minimum rates shall be stated in the invitation for bids and shall be included in proposals or bids for the work.

  • Wage Rates Contractor shall post a copy of the wage rates at the job site and shall pay the adopted prevailing wage rates as a minimum. Pursuant to the provisions of Section 1773 of the Labor Code of the State of California, the Board of Supervisors has obtained the general prevailing rate of per diem wages and the general prevailing rate for holiday and overtime work in this locality for each craft, classification, or type of xxxxxxx needed to execute this Contract from the Director of the Department of Industrial Relations. These rates are on file with the Clerk of the Board of Supervisors. Copies may be obtained at cost at the office of County's OC Public Works/OC Facilities & Asset Management/A&E Project Management or visit the website of the Department of Industrial Relations, Prevailing Wage Unit at xxx.xxx.xx.xxx/XXXX/XXX. The Contractor shall comply with the provisions of Sections 1774, 1775, 1776 and 1813 of the Labor Code.

  • Mileage Rate The mileage rate for authorized travel – based on State of Florida rates on the Effective Date of this Agreement – is $0.445 per mile.

  • Hourly Wage Rates The Employer shall pay wages to every employee covered by this Agreement at the rates set forth in Schedule "A" hereunto annexed in respect of the various classifications therein contained. Schedule "A" shall be deemed to be contained in, and form a part of this Agreement.

  • Supported wage rates Employees to whom this clause applies shall be paid the applicable percentage of the minimum rate of pay prescribed by this Agreement for the class of work which the person is performing according to the following schedule: Assessed Capacity (Clause 1.3) % of prescribed rate 10%* 10% 20% 20% 30% 30% 40% 40% 50% 50% 60% 60% 70% 70% 80% 80% 90% 90% * (Provided that the minimum amount payable shall be not less than $45 per week). Where a person’s assessed capacity is 10%, they shall receive a high degree of assistance and support.

  • Wage Rate The hourly rates for full-time junior and adult apprentices as set out in this agreement shall apply to school based apprentices except that the school based apprentice for pay purposes will be paid a further 25% of hours to the actual hours worked for off the job training.

  • Insurance Availability The teacher will be given an opportunity to continue insurance coverage in the school insurance program during the leave of absence, but will be required to pay all premiums connected with this coverage. All premiums must be paid in advance of the month due. Such premiums shall be prorated on an annual basis from August to August. Teachers initiating leaves of absence shall be charged the prorated amounts during the school year in which their leave begins. As leaves of absence extend into the next school year, the teacher will be expected to pay full premiums.
