Reemployed Participant. If the Participant is reemployed by that Plan Sponsor or any of its Affiliates before he or she incurs a One-Year Break in Service, then the Account Balance in the Forfeiture Suspense Account will be restored to the Participant’s applicable Account or Accounts under the Plan as a Forfeitable Account Balance, and such amount will not be a Forfeiture.

Reemployed Participant. If an Employee satisfies the participation requirements of Section 3.1, terminates employment with an Employer, and is later reemployed by an Employer, the Employee will become a Participant when reemployed, unless upon reemployment, the Employee is not working in Covered Employment, in which case participation shall be delayed until the Employee begins working in Covered Employment.

The custodian named above has given the participant the disclosure statement required by Regulations section 1.408-6. The participant and the custodian make the following agreement:
