Examples of the types of financial fraud identity theft for which claims may be filed include: attempts to open bank accounts, loan applications, new credit card applications, and governmental applications such as FEMA, FAFSA, SSA, and employment benefits. Excluded from eligibility are incidents of fraudulent charges on existing credit cards. If you are seeking reimbursement for an incident of identity theft related to financial fraud, other than the filing of a false/fraudulent tax return or unauthorized request for an IRS transcript, after May 1, 2018, please provide documentation of the fraudulent activity or a signed statement of such activity, including why documentation is not available. Please also attest below that you have no knowledge of incidents of identity theft, other than fraudulent credit card activity, in the three (3) years prior to May 2018. If you spent time dealing with the effects of the Data Incident, please also submit a self- verifying statement of the amount of time you spent that includes documentation demonstrating the time you spent dealing with the effects of the Data Incident. You may xxxx out any transactions that were not fraudulent and any other information that is not relevant to your claim before sending in the documentation. Examples include: You spent at least one full hour calling customer service lines, writing letters or emails, or on the Internet in order to get fraudulent charges reversed or in updating automatic payment programs because your card number changed.
