Common use of EMPLOYEE PREMIUMS Clause in Contracts

EMPLOYEE PREMIUMS. Effective January 1, 2015, employees will have a choice between the Current BCBS of Alabama health insurance with BCBS Point-of-Purchase prescription drug coverage or Optional BCBS of Alabama health insurance with a prescription drug card.

EMPLOYEE PREMIUMS. ‌ Effective January 1, 20152012, employees will have a choice between the Current BCBS of Alabama health insurance with BCBS Point-of-Purchase prescription drug coverage coverage; Current BCBS of Alabama health insurance with Aetna prescription drug card; or Optional BCBS of Alabama health insurance with a Aetna prescription drug card.

EMPLOYEE PREMIUMS. Effective January 1, 20152019, employees will have a choice between the Current BCBS of Alabama health insurance with BCBS Point-of-Purchase prescription drug coverage or coverage, Optional BCBS of Alabama health insurance with a prescription drug cardcard or 100% BCBS plan.
