Continuation of Enrollment. 3.10.1 After the dissolution of an Enrollment Committee, a Person seeking enrollment as a Yukon Indian Person, and a Person making application pursuant to 3.3.2 or 3.3.3 shall apply to the appropriate Yukon First Nation which shall determine, according to this chapter, whether such Person or the Person on whose behalf the application is being made, is entitled to be enrolled under its Yukon First Nation Final Agreement.

Continuation of Enrollment. With respect to the hospital- medical, dental and vision care plans, if an employee is in an unpaid status and not eligible for District contribution, the employee may arrange for continuance of enrollment under COBRA (see 9.0 - 9.3 below).
Continuation of Enrollment. On an annual basis, the screening agent of the MassHealth program will determine whether you are still qualified for nursing facility level of care. (This eligibility screening may not have to be done if the MassHealth screening agents determine that your ongoing medical condition qualifies you for continued enrollment). If the screening agent determines that you no longer meet the criteria for nursing facility level of care, you will not be eligible to continue your enrollment with EB ESP and we must disenroll you from our program. There is one exception to this rule: If the MassHealth screening agents determine through your reassessment that you do meet nursing facility level of care at the time of the assessment, but that without EB ESP services you would be likely to require nursing facility level of care within 6 months, you will be deemed eligible to continue your enrollment with EB ESP.

Disenrollment Your benefits under the Elder Service Plan of the Harbor Health Services can be stopped if you choose to disenroll from the program (voluntarily) or if you no longer meet the conditions of enrollment (involuntarily). Both types of termination require written notice by either party. Disenrollment and termination at any time during the month is effective the first day of the next month. You are required to continue to use ESP of HHSI services and to submit payment, if applicable, until termination of benefits becomes effective.

If, in the opinion of the screening agent, you do not meet the criteria for long term care, you will be deemed ineligible for ESP, and you must disenroll.
