Approval of Students to Participate. Students will gain admission into the CCP program upon University evaluation to ensure that the student fulfills the remediation-free standards and is college-ready. Students will only be considered for CCP admission if all completed materials are submitted to EM by the deadlines posted on the University website and in the College Credit Plus Student Handbook. Students will receive a registration packet from their high school, or may request one from their school's CCP coordinator. School and parent/guardian consent will be required on the Basic Data Form.

Approval of Students to Participate. Based on the student eligibility requirements listed on the Dual Credit New Course Approval Form and the career / academic interests, the District/School will determine students who are eligible to participate. The District/Department shall provide evidence that students meet the eligibility requirements. Students will complete a Dual Credit Request Form indicating the courses (from the list of approved courses) they are requesting. The student and the student’s parent or guardian shall sign the form in acknowledgement of the responsibilities delineated on the form. The District/School representative shall sign the form. An admissions officer from the University shall sign the form.

  • How to File an Appeal of a Prescription Drug Denial For denials of a prescription drug claim based on our determination that the service was not medically necessary or appropriate, or that the service was experimental or investigational, you may request an appeal without first submitting a request for reconsideration. You or your physician may file a written or verbal prescription drug appeal with our pharmacy benefits manager (PBM). The prescription drug appeal must be submitted to us within one hundred and eighty (180) calendar days of the initial determination letter. You will receive written notification of our determination within thirty (30) calendar days from the receipt of your appeal. How to File an Expedited Appeal Your appeal may require immediate action if a delay in treatment could seriously jeopardize your health or your ability to regain maximum function, or would cause you severe pain.
