Optional Form definition

Optional Form means one of the following forms of payment:
Optional Form means a form developed by a Federal agency for use in two or more agencies and approved by GSA for non-mandatory Governmentwide use.

More Definitions of Optional Form

Optional Form means the Actuarial Equivalent of the Normal Form in any form of retirement benefit distribution offered under the SBERA Defined Benefit Plan.
Optional Form means the terms for payment of a pension from the Plan in one of several forms permitted under the Plan, as Elected by the Member under 9.003 ("Optional Forms")S ection 9.03;

The term "Participant" shall include a former employee of the Employer who is entitled to a benefit under the terms of this Supplemental Program. The term "participant" shall not include the Chief Executive Officer of Blue Cross and Blue Shield of Virginia if such Officer and the Employer have made a separate agreement to restore the excess retirement program benefits. 1.9 "Retirement Program" shall mean the funded retirement program specified in the Introduction. 1.10 "Supplemental Program" shall mean the Supplement Retirement Program for Certain Employees of Blue Cross and Blue Shield of Virginia.
Optional Form means a form of benefit available under the Plan other than the Normal Form, consisting of (i) lump sum distribution or (ii) another form of distribution provided under the Contract; provided that such form complies with the requirements of Section 5.09.
