Intensive Care Coordinator definition

Intensive Care Coordinator. (ICC Care Coordinator) means a person coordinating ICC services as defined in this rule.

Intensive Care Coordinator means an individual that has successfully completed specialized training directly related to the Intensive Care Management Model for Service Delivery and meets at a minimum one or more of the following requirements:

  • The CFT, including the Intensive Care Coordinator, makes individualized determinations of each child/youth’s need for ICC and IHBS, based on strengths/needs and reassesses the strengths and needs of child/youth’s, and their families, at least every 90 days and as needed.

  • Care Coordinator means the professional who assists members in care coordination as described in paragraph 78.53(1)“b.”

  • Order Coordination refers to standard BellSouth service order coordination involving the reuse of facilities for SL2 voice loops and all digital loops, where Choctaw is requesting that their loop order be provisioned over an existing circuit that is currently providing service to the end user. Order coordination for physical conversions will be scheduled at BellSouth’s discretion during normal working hours on the committed due date and Choctaw will be advised.

  • Specialist Physician means a licensed physician who qualifies as an attending physician and who examines a patient at the request of the attending physician or authorized nurse practitioner to aid in evaluation of disability, diagnosis, or provide temporary specialized treatment. A specialist physician may provide specialized treatment for the compensable injury or illness and give advice or an opinion regarding the treatment being rendered, or considered, for a patient’s compensable injury.

  • Non-Participating Clinical Professional Counselor means a Clinical Professional Counselor who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Primary care physician means a physician who is a family

  • Licensed professional counselor means a counselor licensed by a Member State, regardless of the title used by that State, to independently assess, diagnose, and treat behavioral health conditions.

  • Title IX Coordinator means an employee of the school district that coordinates the school district’s efforts to comply with and carry out its responsibilities under Title IX. The Title IX Coordinator is responsible for acting as the primary contact for the parties and ensuring that the parties are provided with all notices, evidence, reports, and written determinations to which they are entitled under this policy and grievance process. The Title IX Coordinator is also responsible for effective implementation of any supportive measures or remedies. The Title IX Coordinator must be free from conflicts of interest and bias when administrating the grievance process.
