  • Class Counsel agrees to provide the Settlement Administrator with an executed IRS Form W-9 before the Class Counsel Fees and Class Counsel Costs are issued.

  • The Total Settlement Amount shall be used to pay: (a) Class Counsel Fees; (b) Class Counsel Costs; (c) Settlement Administration Costs; (d) Class Representative Service Awards; and (e) Individual Settlement Payments.
