PROCUREMENT CARD The State has entered into an agreement for purchasing card services. The Purchasing Card enables Authorized Users to make authorized purchases directly from a Contractor without processing Purchase Orders or Purchase Authorizations. Purchasing Cards are issued to selected employees authorized to purchase for the Authorized User and having direct contact with Contractors. Cardholders can make purchases directly from any Contractor that accepts the Purchasing Card. The Contractor shall not process a transaction for payment through the credit card clearinghouse until the purchased Products have been shipped or services performed. Unless the cardholder requests correction or replacement of a defective or faulty Product in accordance with other Contract requirements, the Contractor shall immediately credit a cardholder’s account for Products returned as defective or faulty.
Images If applicable, the CONSULTANT is prohibited from capturing on any visual medium images of any property, logo, student, or employee of the DISTRICT, or any image that represents the DISTRICT without express written consent from the DISTRICT.

As part of the law enforcement services to be provided to CITY, COUNTY 21 has provided, or will provide, mobile data computers (hereinafter called 22 “MDCs”) that are or will be mounted in patrol vehicles and motorcycles, 23 designated by COUNTY for use within CITY limits.
ATM Card If approved, you may use your card and personal identification number (PIN) in automated teller machines (ATMs) of the Credit Union, Instant Cash, and Cirrus® networks, and such other machines or facilities as the Credit Union may designate. For ATM transactions, you must consent to the Credit Union’s overdraft protection plan in order for the transaction amount to be covered under the plan. Without your consent, the Credit Union may not authorize and pay an overdraft resulting from these types of transactions. Services and fees for ATM overdrafts are shown in the document the Credit Union uses to capture the member’s opt-in choice for overdraft protection and the Schedule of Fees and Charges. At the present time, you may use your card to: - Make deposits to your savings and checking accounts. - Withdraw funds from your savings and checking accounts. - Transfer funds from your savings and checking accounts. - Obtain balance information for your savings and checking accounts. - Make point-of-sale (POS) transactions with your card and personal identification number (PIN) to purchase goods or services at POS terminals that carry Instant Cash, and Cirrus® network logo(s). - Access your Overdraft Protection account. The following limitations on ATM Card transactions may apply: - There is no limit on the number of cash withdrawals you may make in any one (1) day. - You may withdraw up to a maximum of $500.00 in any one (1) day, if there are sufficient funds in your account. - There is no limit on the number of POS transactions you may make in any one (1) day. - For security purposes, there are other limits on the frequency and amount of transfers available at ATMs. - You may transfer up to the available balance in your accounts at the time of the transfer. - See Section 2 for transfer limitations that may apply to these transactions. Because of the servicing schedule and processing time required in ATM operations, there may be a delay between the time a deposit (either cash or check) is made and when it will be available for withdrawal.
Catalog Information about Community Regional Medical Center – Diagnostic Medical Sonography Program (CRMC-DMSP) is published in a school catalog that contains a description of certain policies, procedures, and other information about the school. CRMC-DMSP reserves the right to change any provision of the catalog at any time. Notice of changes will be communicated in a revised catalog, an addendum or supplement to the catalog, or other written format. Students are expected to read and be familiar with the information contained in the school catalog, in any revisions, supplements and addenda to the catalog, and with all school policies. Customer Feedback The contractor is expected to establish and maintain professional communication between its employees and customers. The primary objective of this communication is customer satisfaction. Customer satisfaction is the most significant external indicator of the success and effectiveness of all services provided and can be measured through customer complaints and surveys. Performance management drives the contractor to be customer focused through initially and internally addressing customer complaints and investigating the issues and/or problems but the customer always has the option to communicate complaints to the PM, as opposed to the contractor. Customer feedback may also be obtained either from the results of formal customer satisfaction surveys or from random customer complaints. Any customer complaints will be investigated by the PM using the Quality Assurance Monitoring Form – Customer Complaint Investigation, identified in Attachment A4.
