Notify Business Associate Clause Samples
Notify Business Associate in writing and in a timely manner, of any restriction on the Use or Disclosure of PHI that Covered Entity has agreed to or is required to abide by under 45 C.F.R. § 164.522, to the extent that such restriction may impact in any manner the Use and/or Disclosure of PHI by Business Associate under this BAA.
Notify Business Associate. (who will in turn promptly notify whichever of the Recipient or its Subsidiary for which the relevant PHI was created or from which the relevant PHI was received) of any instance of which the person or organization becomes aware in which the confidentiality of such PHI was breached.
