Non-Job Related. The benefit for a non-job-related disability is twenty-five percent (25%) of the employee's average annual compensation, as determined pursuant to Section 9 below. To be eligible for the benefit, the employee must have completed five (5) years of actual and continuous service as a Prince Xxxxxx's County Correctional Officer. Effective June 30, 2001, the benefit for retirement resulting from a non-job-related disability will be thirty percent (30%).

Non-Job Related. Effective July 1, 2001, the benefit for a non-job related disability is equal to a benefit calculated on the regular service formula for a normal benefit pursuant to Section 7.12.B.1., above, but will not be less than thirty percent (30%) of the employee's average annual compensation, as determined pursuant to Section 7.12.G., below. To be eligible for the benefit, the employee must have completed five (5) years of actual and continuous service as a Prince Xxxxxx's County Deputy Sheriff.

  • Surgery Services and Mastectomy Related Treatment This plan provides benefits for mastectomy surgery and mastectomy-related services in accordance with the Women’s Health and Cancer Rights Act of 1998 and Rhode Island General Law 27-20-29 et seq. For the member receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician, physician assistant, or an advance practice registered nurse and the patient, for: • all stages of reconstruction of the breast on which the mastectomy was performed; • surgery and reconstruction of the other breast to produce a symmetrical appearance; • prostheses; and • treatment of physical complications at all stages of the mastectomy, including lymphedema.   • Written Employee Jury Service Policy 1. Unless the Contractor has demonstrated to the County’s satisfaction either that the Contractor is not a “Contractor” as defined under the Jury Service Program (Section 2.203.020 of the County Code) or that the Contractor qualifies for an exception to the Jury Service Program (Section 2.203.070 of the County Code), the Contractor shall have and adhere to a written policy that provides that its Employees shall receive from the Contractor, on an annual basis, no less than five days of regular pay for actual jury service. The policy may provide that Employees deposit any fees received for such jury service with the Contractor or that the Contractor deduct from the Employee’s regular pay the fees received for jury service.
