Letter of Reprimand/Notice of Warning. 1. A reprimand or warning notice may be placed in an employee's personnel file. The employee shall be provided a copy and a notice of an opportunity to reply by registered or certified mail or by personal delivery. The employee shall be given at least ten (10) days to reply before the document is placed in his/her personnel file. The employee's written comments/response, if any, shall be attached to the reprimand or warning notice.
