Individual Family Support Plan. (IFSP) is a written plan developed through a family- centered planning process which identifies the specific concerns, priorities and resources of the family as they relate to the child’s development, and coordinates the services required to meet the needs of the child within the context of the family. The IFSP is the authorizing document for services provided through Part C.
