Home or Office Visits. Patient may request that the Physician see Patient in Patient’s home or office, and in situations where the Physician considers such a visit reasonably necessary and appropriate, the Physician will make every reasonable effort to comply with Patient’s request. If Patient expects all visits (including routine and wellness visits) will be required to be made as Home Visits by the Physician, Patient will select the “Home Visit” Payment Plan.

Similarly, if you are a fee-for-service patient and request a home visit, and it is thought to be reasonably necessary and appropriate, an effort to comply with the request will be made. For non-concierge patients, an additional fee may apply.

Patient may request that the Nurse Practitioner see Patient in Patient’s home or office, and in situations where the Nurse Practitioner considers such a visit reasonably necessary and appropriate, she will make every reasonable effort to comply with Patient’s request (also based on type of membership).

Client may request that the Provider see Client in Client’s home or office, and in situations where the Provider considers such a visit reasonably necessary and appropriate, and the home is within 10 miles of the office, he will make reasonable effort to comply with Client’s request. Client agrees to any additional home or office visit fees.

You can request that a Xxxx Health physician see you in your home or office, and in situations where we consider such a visit reasonably necessary and appropriate, we will make reasonable efforts to comply with your request. If Patient expects all visits by the physician (including routine and wellness visits) to be made as Home Visits, Patient will select the “Home Visit” Payment Plan.
Home or Office Visits. Home visits more than twenty (20) miles from the Physician’s Office will result in an additional fee of $10.00 for each ten (10) miles or part thereof beyond the first twenty (20) miles.

Additional service charge of $100 will be billed to the Patient if the driving distance is less than 20 minutes from the office. The charge will increase to $150 if the driving distance is greater than 20 minutes from the office.
