Health / Medical Care and Action / Management Plans. Medical health care plans for Students will be established by the School with the involvement of the Student's Legal Guardian(s) / Parent(s). Legal Guardian(s) / Parent(s) agree to review these plans annually in conjunction with the School Nurse and ensure any Epi-pen provided to the School remains current at all times. Legal Guardian(s) / Parent(s) will advise the School of any changes to the health, wellbeing or medical condition of Students through its data collection and administration platform soon as reasonably practicable and agree to participate in a revision of the Medical Care or Action Plan for the Student. Legal Guardian(s) / Parent(s) authorise the School to:
