Common use of Extended Health Care and Drug Plan Clause in Contracts

Extended Health Care and Drug Plan. The Employer agrees to pay seventy-five percent (75%) of the billed premiums for such coverage with 10/20 deductible. The Plan provides a dispensing fee cap of five dollars ($5.00) per prescription. The Plan will include vision care providing a payment of two hundred dollars ($200.00) every twenty-four (24) months. In addition, the Plan will pay up to forty dollars ($40.00) every twenty-four (24) months toward the cost of eye exams. Receipts are required for reimbursement. The Plan shall also include para-practitioner coverage at three hundred dollars ($300.00) per para-professional per year.
