Element 3 – Slab Condition Assessment Report. The primary purpose of the Condition Assessment is to ensure that the slab on grade is safe and does not fail under loading. The secondary purpose of the Assessment is to address any serviceability issues such as local damage, unacceptable deformations, and visibly degraded surface conditions such as spalling and cracking. To develop a full understanding of the issues, and fully assess the safety, serviceability, and damaged conditions in the existing building, the Design Team will review Owner provided documents, Geotechnical Report, Slab levelness survey, results of scoping, and proposed repair solutions from mud/foam jacking consultants, any structural analysis, any destructive/non-destructive testing, and visual observations. A Condition Assessment Report will be produced based on qualitative assessment of all the collected data, results of field investigations, testing, and visual observations, and potential repair solutions will be provided based upon engineering experience, and professional judgement.   • ASSESSMENT REPORT Within 120 days following the general election held on November 2nd 2003, the returning officer of the munici- pality shall forward, in accordance with section 659.3 of the Act respecting elections and referendums in munici- palities (R.S.Q., c. E-2.2), an assessment report to the Chief Electoral Officer and the Minister setting out relevant ways to improve the trial and addressing, in particular, the following points : — the preparations for the election (choice of the new method of voting, communications plan, etc.) ; — the conduct of the advance poll and the poll ; — the cost of using the electronic voting system : – the cost of adapting election procedures ; – non-recurrent costs likely to be amortized ; – a comparison between the actual polling costs and the estimated polling costs using the new methods of voting and the projected cost of holding the general election on November 2nd 2003 using traditional methods ; — the number and duration of incidents during which voting was stopped, if any ; — the advantages and disadvantages of using the new method of voting ; — the results obtained during the addition of the votes and the correspondence between the number of ballot paper cards issued to the deputy returning officers and the number of ballot paper cards returned used and unused ; — the examination of rejected ballot papers, if it has been completed.

  • Project Reports; Completion Report 1. The Recipient shall monitor and evaluate the progress of the Project and prepare Project Reports in accordance with the provisions of Section 2.06 of the Standard Conditions and on the basis of indicators agreed with the World Bank. Each Project Report shall cover the period of one (1) calendar semester, and shall be furnished to the World Bank not later than one (1) month after the end of the period covered by such report.

  • Conformity Assessment Procedures 1. Each Party shall give positive consideration to accepting the results of conformity assessment procedures of other Parties, even where those procedures differ from its own, provided it is satisfied that those procedures offer an assurance of conformity with applicable technical regulations or standards equivalent to its own procedures.

  • ADDITIONAL SPECIAL CONTRACT CONDITIONS A. Special Contract Conditions revisions: the corresponding subsections of the Special Contract Conditions referenced below are replaced in their entirety with the following:

  • Final Project Report Prepare a Final Project Report that addresses, to the extent feasible, comments made by the Grant Manager on the Draft Final Project Report. Submit one (1) reproducible master and an electronic copy of the final. Upload an electronic copy of the final report in pdf format to the FAAST system.

  • CMI/RAI MDS Report Recognizing the mutual objective of quality resident care, the Employer agrees to meet through the Union Management Committee with the Union as soon as practicable after the receipt of the annual CMI/RAI MDS report. The Employer agrees to provide the Union with staffing levels, and staffing mix information; the impact of related payroll costs on staffing levels and a written notice of the CMI/RAI MDS report for the facility. The purpose of this meeting is to discuss the impact of the CMI/RAI MDS report on the staffing levels in the Home, quality resident care, and provide the Union with an opportunity to make representation in that regard. The parties shall meet as necessary to discuss other changes or workload issues. The parties may invite additional participants to attend the meeting to support constructive review and discussion.

  • Project Completion Report At the completion of construction and once a Project is placed in service, the Subrecipient must submit a Project Completion Report that includes the total number of units built and leased, affordable units built and leased, DR-MHP units built and leased, an accomplishment narrative, and the tenants names, demographics and income for each DR-MHP unit.

  • Performance Assessment 6.1 The Performance Plan (Annexure A) to this Agreement sets out key performance indicators and competencies that needs to be evaluated in terms of –

  • Risk Assessment An assessment of any risks inherent in the work requirements and actions to mitigate these risks.

  • HHSC SPECIAL CONDITIONS The terms and conditions of these Special Conditions are incorporated into and made a part of the Contract. Capitalized items used in these Special Conditions and not otherwise defined have the meanings assigned to them in HHSC Uniform Terms and Conditions -Grant- Version 2.16.1
