Application for Leave. All requests for leave shall be in writing upon the appropriate form prescribed and provided by the District, with all necessary documentation attached such as physician's statement of incapacity or prepared study program. Requests shall be submitted to the immediate supervisor not less than ten (10) days prior to the beginning date of the leave unless extenuating circumstances do not permit advance notification.
