An Individualized Service Plan. (ISP) is the detailed plan developed by the Child and Family Team that is person centered and includes the child’s treatment goals and objectives, methods of measurement, the timetables to achieve those goals, a description of the services to be provided, the frequency and intensity of each service, and which service providers will provide each service. This term is synonymous with the Wraparound plan of care.
