Non-Participating Clinical Social Worker means a Clinical Social Worker who does not have a written agreement with Blue Cross and Blue Shield of Illinois or another Blue Cross and Blue Shield Plan or Blue Cross Plan to provide services to you at the time services are rendered. COINSURANCE means a percentage of an eligible expense that you are required to pay toward a Covered Service. COMPLICATIONS OF PREGNANCY means all physical effects suffered as a result of pregnancy which would not be considered the effect of normal pregnancy. A normal pregnancy means an intrauterine pregnancy which, through vaginal delivery, results in an infant who is not premature or preterm. CONGENITAL OR GENETIC DISORDER means a disorder that includes, but is not limited to, hereditary disorders, Congenital or Genetic Disorders may also include, but are not limited to, Autism or an Autism Spec- trum Disorder, cerebral palsy, and other disorders resulting from early childhood illness, trauma or injury. COORDINATED HOME CARE PROGRAM means an organized skilled patient care program in which care is provided in the home. Care may be provided by a Hospital’s licensed home health department or by other li- censed home health agencies. You must be homebound (that is, unable to leave home without assistance and re- xxxxxxx supportive devices or special transportation) and you must require Skilled Nursing Service on an intermit- tent basis under the direction of your Physician. This program includes Skilled Nursing Service by a registered professional nurse, the services of physical, occupational and speech therapists, Hospital laboratories, and neces- sary medical supplies. The program does not include and is not intended to providebenefits for Private Duty Nurs- ing Service. It also does not cover services for activities of daily living (personal hygiene, cleaning, cooking, etc.). A ‘‘Plan Coordinated Home Care Program’’ means a Coordinated Home Care Program which has a written agreement with Blue Cross and Blue Shield of Illinois or another Blue Cross and Blue Shield Plan or Blue Cross Plan to provide services to you at the time service is rendered to you. A ‘‘Non-Plan Coordinated Home Care Program’’ means a Coordinated Home Care Program which does not have an agreement with a Blue Cross and Blue Shield Plan or Blue Cross Plan but has been certified as a home health agency in accordance with the guidelines established by Medicare.
