To be eligible for the Community rate your group must be registered as a not for profit organisation or an incorporated group.
Community rate means a rate of payment based on a per member per month capitation rate or its equivalent that applies to a combination of the subscriber groups for a comprehensive medical plan carrier.
The Community rate cell will be paid for individuals who do not meet the state’s nursing home level of care criteria and do not reside in a NF or qualify for an HCBS waiver.
