Unusual Occurrence Report Form. If the matter is not resolved to their satisfaction, the employee may complete an Unusual Occurrence Report Form within seven calendar days of their discussion with the Manager. One report will be forwarded to the Labour/Management Committee for review along with a copy being provided to the xxxxxxx and a further copy being forwarded to the Union.
