Unpaid Leave of Absence for Health Reasons. A. Upon written request of an Instructional Employee (form located in Addendum F), the Board shall grant a leave of absence for a period of not more than two (2) consecutive school years for illness or other disability. A licensed physician shall certify the illness or disability.
