SBA Forms. Completed SBA Forms 480, 652, and 1031 (Parts A and B), showing Borrower’s financial projections (including balance sheets and income and cash flow statements) for the period described therein and a representation to PIOP of Borrower’s intended use of proceeds of the Loans (the “Use of Proceeds Statement”).

SBA Forms. Completed SBA Forms 480, 652, and 1031 (Parts A and B).
SBA Forms. Parent shall have executed and delivered to Lenders SBA Form 1031, PART A of which shall have been completed by Parent, and SBA Forms 652-D and 480.
