ROUTINE ILLNESS. The bank is not intended for use for routine and minor illness when sick leave days are exhausted. Rather, its purpose is to reduce the financial impact of legitimate serious conditions of a teacher member. Medical or recovery needs related to pregnancy and extended care of a family member of a teacher member are included as legitimate uses of the sick leave bank.
ROUTINE ILLNESS. 1. Routine illness is defined as sick leave due to illness or injury of twenty-four (24) consecutively- scheduled work hours duration or less. Paid Time Off For medical reasons will be granted as needed, and only upon evidence which is satisfactory to the employees supervisor. A doctor’s certificate is required for any illness which exceeds three (3) days, and may be requested for shorter absences. Verification by the Employee Health Physician may be required. During short-term absences due to an employees injury or illness, employees will have one hour of accrued Paid Time Off deducted for each hour of the first twenty-four (24) consecutively-scheduled work hours. 2. After this period, the Shod-Tem, Disability Plan and eligibility to use Long Term Illness Bank become effective. Long term illness is defined as sick leave of twenty-four (24) consecutively-scheduled work hours duration. Employees who do not have Long Term Illness Bank time available may utilize their Paid Time Off to make up 100% of their wage (Long Term Illness Bank program is described in Policy IV B)

  • Critical Illness Three (3) days per year, with pay, shall be granted in the case of a critical illness or accident to a member of the employee's immediate family as defined in Section 9.4.2. A statement by the physician verifying the need for the employee to be present with the immediate family member shall be attached to the absence form.

  • Illness injury, or pregnancy-related condition of a member of the employee’s immediate family where the employee’s presence is reasonably necessary for the health and welfare of the employee or affected family member;

  • Extended Illness Sick leave for extended illness (5 or more days) will be paid only during the time period in which a physician certifies the employee to be physically or mentally disabled, and only to the extent of the number of days accumulated.

  • Family Illness The start of a family leave for a serious health condition of a family member shall begin on the date requested by the employee or designated by Management.

  • Medical Verification The Town may require medical verification of an employee’s absence if the Town perceives the employee is abusing sick leave or has used an excessive amount of sick leave. The Town may require medical verification of an employee’s absence to verify that the employee is able to return to work with or without restrictions.
