Requesting Family Leave. In order to request a family leave of absence, the employee must submit a Request for Leave of Absence Form and a Certification of Health Care Provider Form from a physician, private adoption attorney, or state adoption, private adoption or xxxxxx care agency, certifying the need for the leave.
