Optical NIC. ‌ The capability of optical NIC and resources to be configured are modelled with UML and presented in Figure 18, as well as the forwarding rules. Figure 18 – Data Model of Optical NIC Capability: Element <Supported_Work_Mode> identifies the supported working mode of NIC, including Ethernet and TDM mode (sending the packet in time slot way). Element <Mode_Switchover>.indicates that the NIC could dynamically change the working mode without any packet loss. Element <Supported_Actions> specifies the action types which could be merged into the current action set of flow entries of the flow table. Element <Flow_Statistics>, <Port_Statistics> and <Queue_Statistics> define the statistics capability (e.g., byte/bit count) of the Optical NIC which are optional in COSIGN approach since the statistics is a used for traffic engineering purpose, not a basic function. Interface: Port • Element <Supported_Wave> defines the supported wavelength of the optical port or presents as a tunable TX. • Element <State> represents various elements of known state of port and contains two further elements: <oper-state>, <live>. Element <oper-state> represents the reported link state of the port and must have a value of either “up” or “down”. Element <live> must have a value of either “true” or “false”. A value of “true” means the port is active and sending/receiving signals. • Element <Features> contains a list of OpenFlow Port features which contains four sub-lists represented by elements <current>, <advertised>, <supported> and <advertised-peer>. These four lists must contain the features associated with the OpenFlow Port. The specific semantics of feature membership in each of these four sublists are defined in the OpenFlow protocol. • Element <Configuration> identifies several configuration options: <no-receive>, <no-packet- in> and <no-forward>. Element <admin-state> represents the reported admin state of the port and must have a value of either “up” or “down”. These three configuration options must have a value of either “true” or “false”. • Element <Tunnel> enables the association of logical OpenFlow ports with an associated tunnel type and corresponding parameters for the tunnel. • Element <Rate> identifies the current sending/receiving speed of the port.

For the member receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician, physician assistant, or an advance practice registered nurse and the patient, for: • all stages of reconstruction of the breast on which the mastectomy was performed; • surgery and reconstruction of the other breast to produce a symmetrical appearance; • prostheses; and • treatment of physical complications at all stages of the mastectomy, including lymphedema.   • Unbundled Channelization (Multiplexing) 5.7.1 To the extent NewPhone is purchasing DS1 or DS3 or STS-1 Dedicated Transport pursuant to this Agreement, Unbundled Channelization (UC) provides the optional multiplexing capability that will allow a DS1 (1.544 Mbps) or DS3 (44.736 Mbps) or STS-1 (51.84 Mbps) Network Elements to be multiplexed or channelized at a BellSouth central office. Channelization can be accomplished through the use of a multiplexer or a digital cross-connect system at the discretion of BellSouth. Once UC has been installed, NewPhone may request channel activation on a channelized facility and BellSouth shall connect the requested facilities via COCIs. The COCI must be compatible with the lower capacity facility and ordered with the lower capacity facility. This service is available as defined in NECA 4.

  • OPTICAL ‌ The Employer agrees to provide Optical coverage for active full- time employees in the amount of one hundred twenty-five dollars ($125.00) every twenty-four (24) month period, for the cost of frames, lenses and the fitting of prescription glasses when recommended by a doctor or optometrist. Sunglasses or glasses for cosmetic purposes are not included nor is the cost of eye examinations. While this Appendix is intended to provide an overview of all benefit coverages the insurer’s plan documents will ultimately govern the administration of these benefits.   • Electrical appliance safety The Hirer shall ensure that any electrical appliances brought by them to the premises and used there shall be safe, in good working order, and used in a safe manner in accordance with the Electricity at Work Regulations 1989. Where a residual circuit breaker is provided the hirer must make use of it in the interests of public safety.

  • Reactivation To reactivate suspended Service, you must bring your account current through the month of reactivation by making payment in full of any outstanding balance, fees and other applicable charges. In addition, we may require a deposit before reactivating your Service. The amount of the deposit will not exceed one year of monthly fees. Any amounts deposited by you will appear on your statement as a credit, and service charges and other fees will be invoiced as described above. If you fail to pay any amount on a subsequent xxxx, the unpaid amount will be deducted each billing cycle from the credit amount. Credit amounts will not earn or accrue interest.

  • Designated Prescription Drug Prescribers and Pharmacies We may limit your selection of a pharmacy to a single pharmacy location and/or a single prescribing provider or practice. Those members subject to this designation include, but are not limited to, members that have a history of: • being prescribed prescription drugs by multiple providers; • having prescriptions drugs filled at multiple pharmacies; • being prescribed certain long acting opioids and other controlled substances, either in combination or separately, that suggests a need for monitoring due to: o quantities dispensed; o daily dosage range; or o the duration of therapy exceeds reasonable and established thresholds. The Amount You Pay for Prescription Drugs Our formulary includes a tiered copayment structure, which means the amount you pay for a prescription drug will vary by tier. See the Summary of Pharmacy Benefits for your copayment structure, benefit limits and the amount you pay. When you buy covered prescription drugs and diabetic equipment and supplies from a retail network pharmacy, you will be responsible for the copayment and deductible (if any) at the time of purchase. You will be responsible for paying the lower of your copayment, the retail cost of the drug, or the pharmacy allowance. Specialty prescription drugs are generally obtained from a specialty pharmacy. If you buy a specialty prescription drug from a retail network pharmacy, you will be responsible for a significantly higher out of pocket expense than if you bought the specialty drug from a specialty pharmacy. The amount you pay for the following prescription drugs is not subject to the tiered copayment structure: • Contraceptive methods; • Over-the-counter (OTC) preventive drugs; • Nicotine replacement therapy (NRT) and smoking cessation prescription drugs; • Infertility specialty prescription drugs; and • Covered diabetic equipment or supplies bought at a network pharmacy.
