Medicare Part B Beneficiaries. If I am a Medicare Part B beneficiary, or if I will become a Medicare Part B beneficiary at any time within two (2) years after the date of this Agreement, I also agree to the terms listed in the Practice's Medicare Private Contract, and will sign the Practice's Medicare Private Contract in addition to this Agreement to confirm my acceptance of those terms.

Medicare Part B Beneficiaries. If I am a Medicare Part B beneficiary, or if I will become a Medicare Part B beneficiary at any time during the initial one-year term of the Agreement, I also agree to the terms listed in the Medicare Addendum, and will sign the Medicare Addendum in addition to this Agreement to confirm my acceptance of those terms. If I become a Medicare Part B Beneficiary during any renewal term of this Agreement, I agree to sign the Medicare Addendum and submit it to the Practice.
Medicare Part B Beneficiaries. If I am a Medicare Part B beneficiary, or if I will become a Medicare Part B beneficiary at any time during the term of this Agreement, I also agree to enter into the Private Contract as required by the Medicare program. Under the terms of the Private Contact, I cannot submit any bills for Additional Services to the Medicare Program for reimbursement and that any Medicare supplemental insurance may not cover services not paid for by Medicare.

  • Post Retirement Health Care Benefit Employees who separate from State service and who, at the time of separation are insurance eligible and entitled to immediately receive an annuity under a State retirement program, shall be entitled to a contribution of two hundred fifty dollars ($250) to the Minnesota State Retirement System’s (MSRS) Health Care Savings Plan. Employees who have a HCSP waiver on file shall receive a two hundred fifty dollars ($250) cash payment. If the employee separates due to death, the two hundred fifty dollars ($250) is paid in cash, not to the HCSP. An employee who becomes totally and permanently disabled on or after January 1, 2008, who receives a State disability benefit, and is eligible for a deferred annuity under a State retirement program is also eligible for the two hundred fifty dollar ($250) contribution to the MSRS Health Care Savings Plan. Employees are eligible for this benefit only once.

  • Beneficiaries The Executive may designate one or more persons or entities as the primary and/or contingent beneficiaries of any amounts to be received under this Agreement. Such designation must be in the form of a signed writing acceptable to the Board or the Board's designee. The Executive may make or change such designation at any time.

  • Alternate Payee A. Pursuant to the provisions of the Assumption of Liability Endorsement, the Reinsurer has agreed that, in lieu of payment to the Company or its receiver, rehabilitator, liquidator, conservator, or other statutory successor, it shall pay valid claims under the Policy directly to the Insured, at the Insured's request, if a Cut Through Triggering Event (as that term is defined in the Assumption of Liability Endorsement) occurs.

  • Extended Health Care Benefits 12.02(a) The City will provide for all employees by contract through an insurer selected by the City an Extended Health Care Plan which will provide extended health care benefits.
