Health Risk Assessment Entry for SNBC. ‌ The MCO will be responsible for timely entering specified fields from the annual health risk assessment into MMIS for community non-waiver Enrollees, excluding non-waiver Enrollees who have DD. The State will notify the MCO when the MCO must begin entering all initial and annual HRAs for all community Enrollees who are on waivers or receiving DD case management, or Enrollees who live in a nursing facility. The MCO may enter the information or may contract with a Local Agency or Case Management delegate to enter screening documents and shall submit screening documents consistent with the timeframes established by the STATE. The MCO shall submit to the STATE’s security liaison a signed data privacy statement for all MCO employees and subcontractors who will be responsible for entering specified fields from screening documents into MMIS. The STATE shall offer training to MCOs and its subcontractors on this process. The MCO shall download and install the required internet access software “Blue Zone” onto workstations for those staff that will be responsible for entering Screening Documents.
