HEALTH AND SAFETY DECLARATION. In terms of Regulation 5(1)(h) of the OHSA 1993 Construction Regulations 2014 (referred to as ‘the Regulations’ hereafter), a Contractor may only be appointed to perform construction work if the Employer is satisfied that the Contractor has the necessary competencies and resources to carry out the work safely in accordance with the Occupational Health and Safety Act No 85 of 1993, as amended and the OHSA 1993 Construction Regulations 2014. To that effect a person duly authorised by the Tenderer must complete and sign the declaration hereafter in detail. Declaration by Tenderer

HEALTH AND SAFETY DECLARATION. All instruments authorised for return must be accompanied by a Health and Safety Declaration form, completed and signed. This form is enclosed in page 17 of this booklet. Any instruments returned without this form duly completed and signed will be returned without work being carried out and any shipping charges will be carried by the customer.
