Common use of Health and Hospitalization Insurance Clause in Contracts

Health and Hospitalization Insurance. Family Coverage: The School District shall contribute a sum not to exceed $586.00 per month toward the premium for family coverage for each full-time employee employed by the School District who qualifies for and is enrolled in family coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

Family Coverage: The School District shall contribute a sum not to exceed $586.00 10,800 per month year toward the premium for family coverage for each full-time employee teacher employed by the School District who qualifies for and is enrolled in family coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee teacher and paid by payroll deduction.

Health and Hospitalization Insurance. Family Coverage: The School District school district shall contribute a sum not to exceed $586.00 2,350 per month contract year ($195.83 per month) toward the premium for family coverage for each full-time employee employed eligible employee, hired by the School District school district after 6/30/1994, who qualifies for and is enrolled in family coverage in the School District’s school district group health and hospitalization insurance planplan as provided herein. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

Health and Hospitalization Insurance. Family Coverage: The School District shall contribute a sum not to exceed $586.00 per month 10,000.00 annually toward the premium for family coverage for each full-time employee principal employed by the School District who qualifies for and is enrolled in family coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee principal and paid by payroll deduction. The principal shall be eligible to remain in the insurance group upon retirement at the employee’s expense per group policy restrictions and policies until Medicare age or have the opportunity to be covered by another group plan through other employment.

Any additional cost of the premium shall be borne by the employee principal and paid by payroll deduction.
