Evidence of Death. Evidence of death may be requested by Administration. Requests should be submitted to the President/Chief Executive Officer or designee by the Department Head for approval.

Evidence of Death. Age, Marital Status or Survival
Evidence of Death. Age, Gender or Survival We will require proof of death before we act on Contract provisions relating to death of any person or persons. We may also require proof of the age, gender, or survival of any person or persons before we act on any Contract provision dependent upon age, gender or survival. We will not contest this Contract.
Evidence of Death. Age, Sex, or Survival If a Contract provision relates to the death of a person, We will require proof of death before We act under that provision. We will accept a certified death certificate or a certified decree of a court of competent jurisdiction as to a finding of death. We will also accept any other document which is considered due proof of death under applicable state law. If Our action under a Contract provision is based on the age, sex, or survival of any person, We may require evidence of that fact before We act under that provision. However, if the issue state shown on page 2 is Massachusetts or Montana, We will not require evidence of the sex of any person.

  • NOTIFICATION OF DEATH A. Upon becoming aware of the death of any person served pursuant to this Agreement, CONTRACTOR shall immediately notify ADMINISTRATOR.

Whenever a vacancy in the Board of Trustees shall occur, until such vacancy is filled as provided in Article IV, Section 1, the Trustees in office, regardless of their number, shall have all the powers granted to the Trustees and shall discharge all the duties imposed upon the Trustees by this Declaration of Trust.
