Costs for Benefit Plans which are to be borne by the Company The Company shall pay the full premium costs for providing the following benefit coverages to an eligible Regular Employee and, if applicable, his/her eligible dependants:

Costs for Benefit Plans which are to be borne by the Eligible Employees An eligible Regular Employee shall pay the full premium costs for the following benefit coverages, if voluntarily chosen by the Regular Employee for him/herself and, if applicable, his/her eligible dependants: - Voluntary Accidental Death and Dismemberment - Voluntary Life Insurance The Regular Employee’s payment of the premium costs for the above benefit coverages shall be made by way of payroll deduction. However, if there are insufficient monies payable to the Regular Employee in the calendar month to cover the required deduction for the Employee’s payment of the premium cost for the above benefit coverages, it shall be the responsibility of the Employee to ensure that the Company is provided with sufficient funds by the end of the calendar month in order to make the appropriate premium payments for the following month.
