Benefit Plan Document or “BPD” means a prescription drug benefit summary form the Contractor has provided to DCH which, when completed and signed by the SHBP, will describe the essential elements of the applicable prescription drug benefit programs of the SHBP PPO/PPO Consumer Choice and Indemnity health plans.
