Vision Care Providers. The MCOP shall contract with at least the minimum number of ophthalmologists and optometrists for each specified county and region, all of whom shall maintain a full-time practice at a site(s) located in the specified county and region to count toward minimum panel requirements listed in Table 3. All ODM-approved vision providers shall regularly perform routine eye exams. The MCOP will be expected to contract with an adequate number of ophthalmologists as part of its overall provider panel, but only ophthalmologists who regularly perform routine eye exams can be used to meet the vision care provider panel requirement. If optical dispensing is not sufficiently available in a region through the MCOP’s contracting ophthalmologists/optometrists, the MCOP shall separately contract with an adequate number of optical dispensers located in the region.

Vision Care Providers. The ICDS Plan must contract with at least the minimum number of ophthalmologists as required by CMS and ODM and optometrists as required by ODM for each specified county/region, all of whom must maintain a full-time practice at a site(s) located in the specified county/region to count toward minimum panel requirements. All ODM-approved optometrists must regularly perform routine eye exams. If optical dispensing is not sufficiently available in a region through the ICDS Plan’s contracting ophthalmologists/optometrists, the ICDS Plan must separately contract with an adequate number of optical dispensers located in the region.

  • Vision Care Plan The County agrees to provide a Vision Care Plan for all employees and dependents. The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals and with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials. The County will fully pay the monthly premium for the employee and dependents and pick up inflationary costs during the term of the Agreement.
