Urinalysis. c. Eye and ear screenings, provided by a family practi- tioner or general practitioner, for dependent children through age 16 to determine the need for referral to a specialist for eye refraction or audiogram. No benefits are provided for routine examinations by Optometrists or Audiologists, or for routine eye re- fraction.; and
