Common use of Service Reports Clause in Contracts

Service Reports. The Contractor shall be required to provide a signed service report after every service performed under this contract as certification of services performed. Service reports provided shall be inclusive of but not limited to, trouble shooting, repairs, and preventative maintenance. After each site visit, the Contractor shall document in writing the findings of each inspection/preventative maintenance service and any action(s) performed for each item/equipment serviced under this contract. The report shall be submitted to the District’s Maintenance Representatives upon completion of work and prior to leaving the jobsite. For emergency site visit, the report shall be furnished within twenty-four (24) hours of service completion. Individual reports shall be provided to DMR for each service location to include the following: • Technician’s name • Date and time of service (including time-in and time-out) • Location and Facilities Management Asset ID number. • FM work order number • Type of service performed • Result of inspections and recommendations
