Common use of Requests for Leave Clause in Contracts

Requests for Leave. An employee who needs a leave of absence from work shall submit a written request for leave to the Human Resources Department. The request must include the employee’s name, job title, beginning and ending dates for the requested leave, and a statement explaining the reason for the need for leave. The request shall be made on forms prescribed by the District. A request for leave should normally be initiated by the employee but may be initiated by the employee’s supervisor or Department Head.
